The purpose of this questionnaire is to provide a means of regularly and systematically evaluating the effectiveness of our strategic community planning partnerships.

This questionnaire is based on the criteria used within the EFQM Excellence Model and also draws on other examples of good practice around partnership evaluation.

The questionnaire relies on you being open and honest with your individual assessment of partnership arrangements in this evaluation exercise. The overall results of the survey will provide a set of perception measures that we can use with performance indicators for each strategic partnership. This will give a more balanced picture of strengths and potential areas for improvement.
